El presente escrito propone abordar la temática del Parkinson, articulando las nociones de la ciencia en la comprensión de un caso que se presenta en el consultorio del Sector de Salud Mental del Hospital Dr. Joaquín Castellanos en el ejercicio de mi práctica clínica, que se desarrolla en el marco de la Residencia interdisciplinaria con orientación en atención primaria de la salud. Este artículo adquiere la modalidad de aprendizaje basado en el problema, detallando la viñeta clínica, las preguntas orientadoras, la enunciación de la problemática, la búsqueda bibliográfica, el abordaje clínico y las reflexiones finales. A su vez, revela la complejidad de las problemáticas actuales del consultorio que interpelan la práctica clínica y funciona como objeto causa movilizarme a indagar en la temática del parkinson en este caso, evidenciando la importancia del dispositivo psicológico clínico y de su articulación con otras prácticas. This paper addresses the clinical case of a patient recently diagnosed with Parkinson’s disease, treated at the Mental Health Service of Dr. Joaquín Castellanos Hospital, in the province of Salta, Argentina; within the framework of the Interdisciplinary Residency in Primary Health Care (RIAPS). Using the Problem-Based Learning (PBL) methodology, the study proposes a clinical and epistemological reflection on the subjective and social implications of the diagnosis, articulating the patient’s discourse with scientific literature and interdisciplinary practice. The case presents the experience of Parkinson’s as a “living death,” an expression that encapsulates the loss of a healthy and active body, the feeling of uselessness, and the emergence of a depressive state with suicidal ideation. The literature review highlights the high comorbidity between Parkinson’s disease and depression, related both to neurobiological alterations and psychosocial factors. From a psychological standpoint, the intervention focuses on accompanying the symbolic elaboration of mourning for the lost body and fostering the resignification of the new body imposed by the illness. Finally, the paper critically reflects on the limitations of the hegemonic medical model (Menéndez, 1990) and advocates a shift toward the social determinants of health perspective (Pan American Health Organization —PAHO—, 2024), which promotes interventions grounded in complexity, interdisciplinarity, and respect for the subject’s singularity. |
